In order for an alien to file for an Employment Authorization Document, there are certain documents and information that the alien needs to include in the application. In addition to Form I-765, Application for Employment Authorization, the alien applicant must also submit the following documents:

A copy of Form I-94 (front and back),

A copy of previous EADs, if the applicant has had any,

2 photos with a white background taken no earlier than 30 days before submission,

Other supporting documents showing that the applicant is eligible to obtain the EAD (such as I-797 receipt notices, or other documents showing the alien’s non-immigrant status or I-485 application).
